Prior Authorization for Prescribers

Below are resources to manage your patients’ prior authorizations.

INITIATE A PA REQUEST

There are three ways to initiate a Prior Authorization request to Flipt:

• By fax: Complete the General Prior Authorization Intake Form and fax it to Flipt at 1-551-298-4368.
• By email: Complete the General Prior Authorization Intake Form and email it to Flipt at WeCare@fliptrx.com.
• By phone: Call 1-833-354-7879 to initiate a PA request verbally.  Hours of operation are Mon-Fri 8am-8pm ET.

CHECK A PA STATUS REQUEST

To check the status of your PA request, a Concierge advocate will be able to assist you. Please have either the Case ID or your patient’s first name, last name, and date of birth available and call 1-833-354-7879. Hours of operation are Monday through Saturday, 8AM to 8PM Eastern Time.
